Acetaminophen, Phenylpropanolamine falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2E1.
In vitro research shows that goldenseal root extract can inhibit the activity of CYP2E1. However, this effect has not been reported in humans.

Here's the situation with this combo product (which pairs acetaminophen with the decongestant phenylpropanolamine). Your liver uses an enzyme called CYP2E1 to help process a small portion of acetaminophen. In test-tube studies, goldenseal seems to slow that enzyme down. In theory, that could let a bit more acetaminophen linger in your system.

I want to be honest about how thin this evidence is: it comes from lab dishes, not from real people, so we haven't actually seen it happen in patients. The bigger thing I'd watch with acetaminophen isn't goldenseal at all, it's simply staying within the safe daily dose to protect your liver. If you take this regularly, just run it by your pharmacist or doctor so they can give you the all-clear.
