Antithrombin III falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Goldenseal contains berberine. In vitro and animal research shows that berberine can inhibit platelet aggregation. However, this effect has not been reported in humans.

Here's the situation with goldenseal and Antithrombin III (Thrombate III). Antithrombin III is an injectable blood-thinning protein that helps prevent clots, often used around surgery or delivery in people with an inherited antithrombin deficiency. Goldenseal contains berberine, and in test-tube and animal studies berberine seemed to make platelets a little less sticky. Stack that mild blood-thinning hint on top of a medicine whose whole job is to reduce clotting, and the theoretical concern is a slightly higher bleeding risk (think easy bruising, nosebleeds, or bleeding that's slower to stop).

I want to be straight with you: this has not been seen in actual people, so it's more of a caution than a proven problem. Still, since Antithrombin III is used in serious clotting situations, please mention any goldenseal use to your doctor or pharmacist before treatment.
