Aspirin, Omeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the situation with Yosprala, which pairs aspirin with omeprazole. Goldenseal contains alkaloids (berberine and hydrastine) that, in test-tube studies, can mildly slow down a liver enzyme called CYP2C9. Since this enzyme helps clear part of this medication, the theory is that goldenseal could let levels build up a little, which might mean a bit more aspirin effect (think easier bruising, stomach upset, or minor bleeding).

I want to be honest though: this has only been seen in lab studies, never confirmed in real people, so the real-world risk looks low. It's not something to lose sleep over, but if you take Yosprala daily for your heart and are thinking about goldenseal supplements, just run it by your pharmacist or doctor first.

Aspirin, Omeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist with Yosprala, which combines aspirin and omeprazole. The omeprazole part is partly broken down by a liver enzyme called CYP3A4. Goldenseal can slow that enzyme down, so in theory omeprazole could hang around at slightly higher levels in your body. For a stomach acid reducer like this, that usually isn't a dramatic change, but it's the piece that could be nudged.

The aspirin half works on your platelets and isn't really affected by this pathway, so goldenseal isn't expected to change its blood-thinning action. The evidence here is moderate and based on small studies, so I wouldn't panic. Still, since goldenseal supplements are much more concentrated than anything you'd eat, it's worth a quick chat with your pharmacist or doctor before combining them.

Here's the gist with Yosprala, which pairs aspirin (a blood thinner that keeps platelets from clumping) with omeprazole (which calms stomach acid). Goldenseal contains berberine, and in lab dishes and animal studies berberine also seems to make platelets less sticky. Since aspirin already works by quieting platelets, the worry is that stacking the two could nudge your bleeding risk a little higher.

I want to be honest about how soft this evidence is: this effect has only been seen in test tubes and animals, not in actual people. So it's more of a theoretical caution than a proven problem. Still, watch for things like easy bruising, nosebleeds, bleeding gums, or dark stools. Before adding goldenseal, run it past your pharmacist or doctor first.
