Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

Here's the gist: Apadaz (benzhydrocodone with acetaminophen) is a strong opioid pain reliever that can already make you drowsy, foggy, or slow your breathing. Goldenseal contains berberine, and in animal studies berberine showed some sedative, calming effects. The worry is that stacking the two could deepen that drowsiness.
I want to be honest with you, though. This concern comes from lab and animal work only, and that sedative effect has not actually been seen in people. So the evidence here is theoretical and fairly weak. Still, since an opioid is involved, I wouldn't combine them casually. If you take more than the occasional sleepiness, extra grogginess, or feel unusually out of it, give your pharmacist or doctor a quick call before continuing both.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 4Benzhydrocodone, Acetaminophen also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the tricky part with Apadaz. The benzhydrocodone piece is actually a prodrug. After you swallow it, your body has to convert it into hydrocodone, the part that actually relieves pain. Hydrocodone is then further processed by the CYP2D6 enzyme into hydromorphone, an even stronger active form.
Goldenseal can block CYP2D6. Because of that, this gets a little complicated: some active hydrocodone may build up while less is turned into hydromorphone, so the net painkilling effect can shift in either direction. If levels run higher than expected, you could see more drowsiness, constipation, nausea, or slowed breathing. The evidence here is moderate, not rock solid. Before pairing the two, please check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's something worth knowing about Apadaz (benzhydrocodone with acetaminophen). The benzhydrocodone part is actually a prodrug, meaning your body has to convert it into hydrocodone before it can ease your pain. That activation leans on certain enzymes, and the painkilling hydrocodone is then further handled by CYP3A4, the same enzyme goldenseal tends to block.
Because goldenseal slows CYP3A4, it could let active opioid hang around longer in your system. In plain terms, that may mean stronger or longer-lasting effects like heavy drowsiness, dizziness, constipation, or slowed breathing. The evidence here is moderate and not certain for this exact combo, so don't panic. Just check with your pharmacist or doctor before pairing them, especially since this is an opioid.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In vitro research shows that goldenseal root extract can inhibit the activity of CYP2E1. However, this effect has not been reported in humans.

Here's the practical picture with Apadaz (benzhydrocodone and acetaminophen) and goldenseal. The acetaminophen part is partly broken down by an enzyme called CYP2E1. In test-tube studies, goldenseal seems to slow that enzyme down a little. In theory, that could mean acetaminophen lingers longer in your system. The good news is this has not actually been shown to happen in people, so it's a fairly weak, lab-only concern.
One thing worth knowing: that same CYP2E1 pathway is what can turn acetaminophen into a liver-stressing byproduct, so the practical risk here is more about the bigger acetaminophen picture than goldenseal itself. Keep your total daily acetaminophen in check, avoid heavy alcohol, and don't double up on other Tylenol-containing products. If you want to use goldenseal while taking Apadaz, just run it by your pharmacist or doctor first.

What Is Benzhydrocodone and Acetaminophen Used For?
Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
