Here's the thing with goldenseal and your Rexulti (brexpiprazole). Your body relies heavily on a liver enzyme called CYP2D6 to break down and clear this medication. Goldenseal can block that enzyme, and research suggests it does this fairly strongly. When that pathway slows down, brexpiprazole isn't cleared as quickly, so the amount in your blood can creep up over time.

In practical terms, that could mean more of the usual side effects you might feel from this drug, things like drowsiness, restlessness or feeling fidgety, dizziness, weight changes, or a sense of being overly sedated. Food amounts aren't really the concern here, it's concentrated goldenseal supplements. Please don't change anything on your own, just check with your pharmacist or prescriber first.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for Rexulti (brexpiprazole): your liver uses an enzyme called CYP3A4 to break it down and clear it out. Goldenseal appears to slow that enzyme down. When the cleanup slows, more brexpiprazole can hang around in your blood, which means the drug's effects (and side effects) can get stronger than expected.

For this medication, that could show up as extra drowsiness, dizziness, restlessness, weight changes, or feeling foggy. The evidence here is moderate and the effect is possible rather than guaranteed, but I wouldn't combine them on your own. Please check with your pharmacist or doctor first so they can decide if your dose or routine needs a closer look. Casual amounts aren't the concern here, concentrated goldenseal supplements are.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
