Here's the practical piece for Dimetapp DM. One of its active ingredients, dextromethorphan (the cough suppressant), is cleared from your body mainly by a liver enzyme called CYP2D6. Goldenseal is known to slow that enzyme down, so the dextromethorphan can build up higher than usual. In real life, that often shows up as extra drowsiness, dizziness, or feeling foggy, and in some people, restlessness or confusion.

The brompheniramine (antihistamine) can also lean on this same enzyme, so sleepiness may stack up too. This is rated a moderate, likely interaction, but it isn't an emergency. Before pairing goldenseal with this product, I'd just run it by your pharmacist or doctor so you can weigh it out together.

Goldenseal contains berberine. A small clinical study shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan.

This combo product, sold as Dimetapp DM, includes dextromethorphan, the cough suppressant. Your body clears dextromethorphan mainly through a liver enzyme called CYP2D6. Goldenseal contains berberine, which a small study suggests can slow that enzyme down. In theory, that means dextromethorphan could build up a bit higher than usual.

If that happens, you might notice more of its typical effects, like drowsiness, dizziness, or feeling foggy. Keep in mind this is a theoretical concern based on how the drugs work, not solid proof it causes problems. Still, since the brompheniramine in this product can also make you sleepy, it's worth a quick chat with your pharmacist before pairing it with goldenseal supplements.
