Of the three ingredients in this combo cough and cold product, the one that matters here is the dextromethorphan (the DM cough suppressant). Your body clears it using a liver enzyme called CYP2D6. Goldenseal can slow that enzyme down, so dextromethorphan may not break down as quickly and can build up in your system.

In plain terms, you might feel more of its effects than usual, things like drowsiness, dizziness, or feeling foggy or lightheaded. The guaifenesin and pseudoephedrine aren't really affected by this pathway. The evidence here is moderate, not alarming, but real enough to mention. Occasional, short-term use of a cough product is lower risk, but please check with your pharmacist or doctor before pairing concentrated goldenseal supplements with this medicine.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Of the three ingredients in this cough and cold combo, dextromethorphan is the one that matters here. Goldenseal can slow down a liver enzyme (CYP3A4) that helps break dextromethorphan down. When that pathway is blocked, more of the drug can linger in your system. Worth noting too: dextromethorphan also leans heavily on a second enzyme (CYP2D6), so the real-world impact may be smaller than it sounds.

If levels do climb, you might feel more drowsy, dizzy, or lightheaded, sometimes with nausea. The guaifenesin and pseudoephedrine parts aren't really affected by this. The evidence is moderate, not alarming, so don't stop anything on your own. Just check with your pharmacist or doctor before pairing goldenseal supplements with this product.

This combo cough and cold product has three ingredients, but the one that matters here is the dextromethorphan (the cough suppressant). Your liver clears it using an enzyme called CYP2D6. Goldenseal contains berberine, which can slow that enzyme down. In theory, that means dextromethorphan could build up in your body a bit more than usual.

If levels climb, you might notice more of dextromethorphan's typical effects, like drowsiness, dizziness, or feeling foggy. I'll be honest though: this is based mostly on how the drugs behave on paper and one small study, not strong real-world data, so it's a possible concern rather than a guaranteed problem. The guaifenesin and pseudoephedrine pieces aren't really affected. Just give your pharmacist a heads-up before pairing them so we can keep an eye on it.
