Enasidenib falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goldenseal interacts with.
Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist: goldenseal can block a liver enzyme called CYP2D6, and your body uses that same pathway to help clear enasidenib (Idhifa). When that exit route slows down, more of the drug can hang around in your blood than expected. Enasidenib is the active drug itself, not a prodrug, so higher levels generally mean a stronger effect and a greater chance of side effects.
For enasidenib that could mean more nausea, diarrhea, fatigue, or higher bilirubin (a lab change your team watches). Because this is a serious cancer medicine with careful dosing, even modest shifts matter. The evidence here isn't airtight, but to play it safe, please check with your oncologist or pharmacist before adding goldenseal.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP2C9 to help clear enasidenib (Idhifa). Goldenseal contains alkaloids (berberine and hydrastine) that may gently slow that enzyme down. If that happens, enasidenib could build up a bit more than expected, which in theory might mean more of its known side effects, like nausea, diarrhea, low blood counts, or rises in bilirubin.
That said, this concern comes only from lab dish research, not from real people, so we honestly don't know if it matters in everyday life. Because enasidenib is a serious cancer medicine, I wouldn't add goldenseal on your own. Please check with your oncology team or pharmacist first so they can weigh in.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Enasidenib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to help clear enasidenib (Idhifa). Goldenseal tends to slow that enzyme down. When that happens, this leukemia drug can be cleared less efficiently, so more of it may linger in your bloodstream than intended.
Why does that matter? Higher enasidenib levels could mean a greater chance of its known side effects, like nausea, diarrhea, elevated bilirubin, or a serious reaction called differentiation syndrome. Most of the evidence here comes from how goldenseal affects other CYP3A4 drugs, not enasidenib directly, so this is a reasonable concern rather than a proven problem. With a cancer medicine this important, please don't change anything on your own. Check with your oncologist or pharmacist before combining the two.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
