Levorphanol is a strong opioid pain reliever, and one of its main effects is slowing down the brain and nervous system. That can mean drowsiness, dizziness, slowed breathing, and feeling foggy. Goldenseal contains berberine, and in animal studies berberine showed some calming, sedative-like effects. The concern here is simple: piling a possible sedative on top of an opioid could, in theory, leave you more sleepy or sluggish than the opioid alone.

I want to be honest about how soft this evidence is. This sedation has not actually been reported in people, so it's a theoretical caution, not a proven problem. Still, since levorphanol is potent, it's worth mentioning goldenseal to your pharmacist or doctor before combining them, and watching for extra drowsiness if you do.
