Theoretically, goldenseal might increase the sedative effects of CNS depressants.
Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

Meprozine and Mepergan combine meperidine (a narcotic pain reliever) with promethazine (an antihistamine that already makes most people drowsy). On their own, this pairing tends to cause sleepiness, slowed breathing, dizziness, and a foggy head. The concern here is simple: goldenseal contains berberine, which in animal studies showed mild sedative effects, so stacking it on top could, in theory, add to that drowsiness.

I want to be honest with you, though. This is a theoretical interaction based on lab and animal work, and the extra sedation has not actually been reported in people. Still, since this medication is already quite sedating, it's worth mentioning. Please check with your pharmacist or doctor before combining them.

Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.

Here's the thing with this combo pill (Meperidine plus Promethazine): goldenseal has been shown to slow down a liver enzyme called CYP2D6, which helps your body clear both of these drugs. When that enzyme is blocked, the medications can hang around longer and build up to higher levels than usual.

In practice, that could mean stronger effects from both pieces: more drowsiness, dizziness, slowed breathing, confusion, and low blood pressure. Since meperidine is an opioid and promethazine is sedating on its own, piling extra sedation on top is the main worry. This is based on lower quality human research, so it's a real but not certain concern. Please check with your pharmacist or doctor before combining them rather than changing anything yourself.
