Why this applies to Modafinil

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for Modafinil (Provigil): your body partly relies on a liver enzyme called CYP3A4 to break it down. Goldenseal tends to slow that enzyme down, so the medication may clear a bit more slowly and build up in your system. In plain terms, you could feel modafinil's effects more strongly than usual.

That might mean more headache, jitteriness, trouble sleeping, a faster heartbeat, anxiety, or nausea. This isn't a guaranteed reaction, and the evidence here is moderate rather than rock-solid, but it's worth taking seriously. Keep in mind concentrated goldenseal capsules are far more likely to matter than a tiny culinary amount. Before combining these, please check in with your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES

Modafinil also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, goldenseal might increase or decrease serum levels of P-glycoprotein (P-gp) substrates.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.

Here's the gist for goldenseal and modafinil. Modafinil is handled in part by a transporter pump called P-glycoprotein, and goldenseal might nudge that pump in either direction. The honest answer is that the evidence is shaky and mostly theoretical. In real human testing, goldenseal only changed levels of another pump drug (digoxin) by a small amount, so it doesn't look like a strong blocker.

For you, this means any effect on modafinil is likely to be minor. In theory, if levels crept up a bit, you might notice more jitteriness, headache, or trouble sleeping; if they dipped, modafinil could feel a little less effective. I wouldn't lose sleep over it, but please run it by your pharmacist or doctor before combining the two.

What Is Modafinil Used For?

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
