Why this applies to Nimodipine

Nimodipine falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the situation with goldenseal and nimodipine. Nimodipine is a calcium channel blocker that relaxes blood vessels, so it can lower your blood pressure on its own. Goldenseal contains berberine, which animal studies suggest may also nudge blood pressure down. Stacking the two together could, in theory, drop your blood pressure a bit more than expected.

If that happened, you might feel lightheaded, dizzy, or faint, especially when standing up quickly. I'll be honest though: this concern is theoretical, based mostly on lab and animal data, and it hasn't actually been reported with goldenseal in people. Nimodipine is also a serious medicine used after a brain bleed, so don't add or stop anything on your own. Please run goldenseal by your doctor or pharmacist first.

Nimodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the thing to know about combining goldenseal with nimodipine. Your body relies on an enzyme called CYP3A4 to break nimodipine down, and goldenseal tends to slow that enzyme down. When that happens, more nimodipine can linger in your bloodstream than expected, which essentially boosts its blood-vessel-relaxing effect.

For nimodipine that mostly shows up as a bigger drop in blood pressure, so you might feel dizzy, lightheaded, flushed, or notice a faster heartbeat. Since nimodipine is given in a delicate situation (after a brain bleed), steady blood pressure really matters here. The evidence for goldenseal blocking this enzyme is fairly reasonable, but I wouldn't pair the two without checking with your doctor or pharmacist first.

Nimodipine — available as capsules and oral solution — is used to improve neurological outcomes after a subarachnoid hemorrhage (SAH), a type of bleeding that occurs around the brain when an intracranial berry aneurysm ruptures. It works to reduce the incidence and severity of ischemic deficits (areas of the brain damaged by reduced blood flow) that can follow this kind of bleed.

This use applies across nimodipine capsules, nimodipine oral solution, and Nymalize oral solution. It is indicated for adult patients regardless of how severe their neurological condition is at the time treatment begins.
