Why this applies to Siponimod

Siponimod falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the situation with goldenseal and your Mayzent (siponimod). Your body clears siponimod largely through a liver enzyme called CYP2C9. Goldenseal, in test-tube studies only, appears to mildly slow that enzyme down. If that were to happen in real life, siponimod could clear more slowly and build up a little, which might mean stronger effects like a lower heart rate, raised blood pressure, headaches, or a greater drop in your immune defenses and risk of infection.

The reassuring part: this has only been seen in lab studies, never confirmed in people, so it's a theoretical concern. Still, siponimod is a drug where blood levels really matter, so please check with your pharmacist or doctor before adding goldenseal rather than starting or stopping anything on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Siponimod also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to help clear siponimod (Mayzent) out of your system. Goldenseal tends to slow that enzyme down. When that happens, siponimod can hang around longer than intended and build up in your blood, which means more drug effect than your dose was meant to give.

With siponimod, higher levels can mean a stronger drop in your heart rate, especially when you first start it, plus more risk of infections, headaches, or changes in liver tests or blood pressure. This is a possible interaction, not a guaranteed one, but because siponimod is carefully dosed, I'd play it safe. Please check with your pharmacist or MS doctor before pairing goldenseal with it rather than changing anything yourself.

Mayzent (siponimod) is approved for adults with relapsing forms of multiple sclerosis (MS). This includes three specific types: clinically isolated syndrome (a first episode of MS-like symptoms), relapsing-remitting MS (periods of attacks followed by recovery), and active secondary progressive MS (a phase where the disease gradually worsens but still has active relapses).

It is taken by mouth once daily and is intended as a long-term treatment to reduce relapses and slow disease progression.
