Why this applies to Vibegron

Vibegron falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for Gemtesa (vibegron): goldenseal tends to block a liver and gut enzyme called CYP3A4, which is one of the pathways your body uses to clear vibegron. When that pathway is slowed down, more of the drug can hang around in your bloodstream than usual.

Vibegron is an active medication, not a prodrug, so higher levels generally mean a stronger effect and a bit more chance of side effects like headache, dry mouth, constipation, or a small rise in blood pressure. The evidence here is moderate and based mostly on other CYP3A4 drugs, not vibegron itself, so this is a real but not alarming concern. Occasional culinary amounts matter less than concentrated capsules. Before pairing them, check with your pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES

Vibegron also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, goldenseal might increase or decrease serum levels of P-glycoprotein (P-gp) substrates.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.

Here's the short version on mixing goldenseal with Vibegron (Gemtesa). Vibegron is carried around the body partly by a transporter called P-glycoprotein, and goldenseal can nudge that transporter a little. The tricky part is that the research goes both ways: some lab work suggests goldenseal mildly blocks this transporter, while other studies suggest it speeds it up. In real people, the effect has been small (for example, only about a 14% bump in peak levels of a similar drug, digoxin).

So in practice, goldenseal might slightly raise Vibegron levels and bring on more headache, dry mouth, or constipation, or it could blunt how well it calms your bladder. The effect is likely minor, but since this is theoretical, please run it by your pharmacist or doctor first.

Gemtesa (vibegron) is used to treat overactive bladder (OAB) in adults. Specifically, it targets the three main OAB symptoms: sudden strong urges to urinate, leaking urine when you feel that urge (urge urinary incontinence), and needing to urinate too often.

Gemtesa is also approved for adult men who have OAB symptoms and are already taking medication for benign prostatic hyperplasia (BPH) — an enlarged prostate.
