Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the situation with goldthread and your apalutamide (Erleada). Goldthread is rich in berberine, and in early lab work berberine seems to slow down the liver enzyme CYP3A4, which is one of the pathways that helps clear apalutamide from your body. In theory, blocking that pathway could let apalutamide build up a bit higher, which might mean more of its usual side effects like fatigue, joint pain, rash, high blood pressure, or, in rare cases, seizures.

I want to be honest though: this is theoretical. It's based on test-tube findings, not real cases in people, so the actual risk is uncertain. Apalutamide is also a strong enzyme inducer itself, which complicates things. Still, since this is serious cancer therapy, please don't add goldthread or berberine without first checking with your oncology team or pharmacist.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
