Goldthread and Elagolix, Estradiol Norethindrone Acetate, Elagolix

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Elagolix, Estradiol Norethindrone Acetate, Elagolix

Elagolix, Estradiol Norethindrone Acetate, Elagolix falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

If you're taking Oriahnn (elagolix with estradiol and norethindrone) for fibroid bleeding, here's the gist. The elagolix part is partly cleared by a liver enzyme called CYP3A4, and the estrogen and progestin pieces lean on it too. Goldthread contains berberine, which in lab studies may slow that same enzyme down. In theory, that could let these hormones build up a bit, which might mean more side effects like headaches, hot flashes, nausea, mood changes, or breast tenderness.

I want to be honest: this is theoretical, based on test-tube data, and hasn't actually been reported in people. Concentrated berberine supplements are more of a concern than a casual sip of tea. Still, since this is a multi-hormone medicine, I'd run it past your pharmacist or doctor before combining them.
