Letrozole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Your body clears letrozole partly through a liver enzyme called CYP3A4. Goldthread contains berberine, which lab studies suggest might slow that same enzyme down. If that happened, letrozole could clear more slowly and build up a bit higher than usual. With letrozole, that might mean more of its typical effects, like hot flashes, joint and muscle aches, tiredness, or headaches.

Here's the reassuring part: this is only a theoretical concern based on how berberine behaves in test tubes, not something actually reported in people. Still, because letrozole is treating something as serious as breast cancer, I'd keep it simple and check with your oncologist or pharmacist before adding goldthread rather than guessing.

What Is Letrozole Used For?

Brand name: Femara

Letrozole (brand name Femara, or available as generic letrozole tablets) is used to treat breast cancer in postmenopausal women whose tumors are hormone receptor positive — meaning the cancer feeds on estrogen. It is used in several situations: as an adjuvant treatment (therapy given after primary treatment to reduce the risk of the cancer returning) for early breast cancer, and as extended adjuvant treatment for women who have already completed five years of tamoxifen therapy.

Femara and letrozole tablets are also used as first-line or second-line treatment for locally advanced or metastatic (spread to other areas) breast cancer in postmenopausal women, including cases where the cancer has progressed after other anti-estrogen therapy.
