Naldemedine Tosylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the situation with Symproic (naldemedine) and goldthread. Your body clears naldemedine using a liver enzyme called CYP3A4. Goldthread contains berberine, which lab studies suggest might slow that same enzyme down. If that happened, naldemedine could clear more slowly and build up a bit higher than usual. Since naldemedine is an active drug (not a prodrug), more of it means a stronger effect, which could show up as belly cramps, diarrhea, nausea, or stomach pain.

The good news: this is only a theoretical concern based on lab work, and it hasn't been reported in actual people. Still, it's worth a quick chat with your pharmacist or doctor before combining them, so you can keep an eye out for any extra stomach upset.

What Is Naldemedine Used For?

Brand name: Symproic

Symproic (naldemedine) is used to treat opioid-induced constipation (OIC) — constipation caused by opioid pain medications — in adults with chronic non-cancer pain. This includes people living with chronic pain related to prior cancer or its treatment, as long as they do not need frequent opioid dose increases (such as weekly adjustments).

It is not a general constipation remedy. Symproic works specifically against the constipating effect of opioids and should be stopped if you stop taking your opioid pain medication.
