Why this applies to Osimertinib Mesylate

Osimertinib Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Your body relies on a liver enzyme called CYP3A4 to help break down osimertinib (Tagrisso). The berberine in goldthread may slow that same enzyme down a bit. In theory, that could let osimertinib build up to higher levels than expected, which might mean more side effects like diarrhea, mouth sores, rash, dry or itchy skin, fatigue, or changes in how your heart's rhythm beats (a QT effect this drug is already known for).

I want to be honest about the evidence though. This is based on lab work, not on real cases in people, so it's more of a caution than a proven problem. Since osimertinib is a serious cancer therapy, please don't add goldthread or berberine without first checking with your oncology team or pharmacist.

About Osimertinib

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
