Why this applies to Pexidartinib Hydrochloride

Pexidartinib Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the simple version: your body uses an enzyme called CYP3A4 to break down pexidartinib (Turalio) and clear it out. Berberine, the active compound in goldthread, may slow that same enzyme down. If pexidartinib isn't cleared as quickly, its levels could creep up, which can mean stronger side effects like nausea, fatigue, hair color changes, swelling, or, importantly, liver strain. Pexidartinib already carries a serious liver warning, so that's the part I'd watch most.

That said, this is a theoretical concern based on lab studies, not something reported in real patients. Still, with a cancer drug this carefully dosed, I wouldn't combine them without checking first. Please talk with your oncologist or pharmacist before adding goldthread.
