Polatuzumab Vedotin-Piiq falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist with goldthread and Polivy. Goldthread contains berberine, which lab work suggests might slow down the CYP3A4 enzyme. Polivy releases a chemo payload (called MMAE) that the body clears partly through CYP3A4. In theory, blocking that enzyme could let more of the active drug linger, which might mean stronger side effects like nerve tingling or numbness in the hands and feet, low blood counts, fatigue, or a higher infection risk.

I want to be honest: this is theoretical, based on pharmacology rather than real patient reports. Still, Polivy is serious cancer therapy, so I wouldn't add goldthread on your own. Please run it by your oncology team or pharmacist first so nothing complicates your treatment.

What Is Polatuzumab vedotin-piiq Injection Used For?

  • Polatuzumab vedotin-piiq injection is used along with bendamustine (Belrapzo, Treanda) and rituximab (Rituxan) in adults to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not improve or improved but returned after treatment with at least two other chemotherapy medications. Polatuzumab vedotin-piiq is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
