There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the thing to know about riluzole: it can be hard on the liver on its own. That's why your doctor checks your liver blood tests regularly while you're taking it. Gotu kola has been tied to a handful of liver-injury reports too, though we honestly can't say for sure it was the cause. The worry is simply that two things that can stress the liver might add up.

This isn't about gotu kola changing how riluzole works in your body, it's about a possible extra burden on the liver. Watch for signs like yellowing skin or eyes, dark urine, belly pain, unusual tiredness, or nausea. The evidence here is weak and mostly anecdotal, so don't panic, but please run it by your pharmacist or neurologist first.

What Is Riluzole Used For?

Brand names: Rilutek, Tiglutik, Exservan

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
