Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Apadaz (benzhydrocodone with acetaminophen) and grape products. The grape connection here works through a liver enzyme called CYP1A2. In one small study, grape juice seemed to speed up that enzyme a bit, which could lower the levels of medicines it processes. It's worth knowing that benzhydrocodone is a prodrug, meaning your body has to convert it into its active opioid form to get pain relief.
The honest takeaway is that this is mostly theoretical and based on weak evidence, so I wouldn't lose sleep over it. Everyday grapes in your diet are very unlikely to matter. If you're considering a concentrated grape seed extract supplement, just run it by your pharmacist or doctor first so they can keep an eye on your pain control.

In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.

Here's the wrinkle with Apadaz: the benzhydrocodone part is actually a prodrug. Your body has to convert it into hydrocodone, and then an enzyme called CYP2D6 turns some of that into hydromorphone, which adds to the pain relief. So this isn't a simple 'levels go up' situation.
The concern on file is that concentrated grape seed extract might block CYP2D6 in a test tube. If that happened in a person, it could mean a little less of the active pain-relieving form, so the medicine might not work quite as well. Honestly though, this is theory based only on lab data, never shown in real people, and eating grapes won't do this. Still, if you're using a grape seed supplement, give your pharmacist a quick heads-up.

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the thing about Apadaz: the benzhydrocodone part is actually a prodrug. Your body has to convert it into hydrocodone before it does anything for your pain. So the direction of any interaction matters in a special way. If a grape seed supplement sped up the CYP3A4 enzyme, you might get a bit less pain relief. If it slowed that enzyme, the opioid could build up and you'd see more drowsiness, dizziness, constipation, or slowed breathing.
The honest answer is that the lab and animal research disagrees on which way grape seed extract pushes, and none of this has actually shown up in people. Eating grapes is no concern at all. If you take a concentrated supplement, just give your pharmacist a heads-up so we can keep an eye on things.

Here's the gist with Apadaz (benzhydrocodone and acetaminophen) and grape seed extract. The acetaminophen part of this combo is partly processed by a liver enzyme called CYP2E1. Lab and animal studies hint that concentrated grape seed extract might slow that enzyme down. In theory, slowing it could shift how acetaminophen is handled, but this has never actually been shown to happen in people, so I wouldn't lose sleep over it.
A couple of practical notes: eating grapes or drinking a little grape juice is not the concern here, we're talking about concentrated supplement doses. What Is Benzhydrocodone and Acetaminophen Used For?
Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
