Brentuximab Vedotin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the situation with grape seed extract and Adcetris (brentuximab vedotin). The active piece of this medicine, called MMAE, is partly broken down by a liver enzyme known as CYP3A4. Grape seed extract may nudge that enzyme, but the data is genuinely mixed: lab (test tube) studies hint it might slow the enzyme down, while animal studies suggest it could speed it up. None of this has actually been seen in real patients, so it's theoretical right now.

If grape seed extract did slow that enzyme, more MMAE could build up, which might worsen common side effects like nerve tingling or numbness, low blood counts, fatigue, or nausea. If it sped the enzyme up, the drug could clear faster. Since brentuximab is serious chemotherapy, please don't add a grape seed supplement without first checking with your oncology team. Eating grapes is fine.

What Is Brentuximab Vedotin Injection Used For?

Brand name: Adcetris

Adcetris (brentuximab vedotin) is used to treat classical Hodgkin lymphoma (cHL) in adults — including newly diagnosed advanced-stage disease, as consolidation therapy after a stem cell transplant, and in relapsed or refractory cases. It is also approved for pediatric patients aged 2 and older with previously untreated high-risk classical Hodgkin lymphoma, given in combination with chemotherapy.

Adcetris is also used for several T-cell lymphomas — including systemic anaplastic large cell lymphoma (sALCL), peripheral T-cell lymphomas (PTCL), primary cutaneous anaplastic large cell lymphoma (pcALCL), and CD30-expressing mycosis fungoides (MF). Additionally, Adcetris is used in combination with lenalidomide and a rituximab product for adults with relapsed or refractory large B-cell lymphoma (LBCL) who are not candidates for a stem cell transplant or CAR T-cell therapy.
