Fomoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.

Theoretically, grape seed extract may increase the levels of CYP2D6 substrates.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.

Here's the gist with grape seed extract and your formoterol (Foradil). Formoterol is partly handled by a liver enzyme called CYP2D6. Lab studies suggest concentrated grape seed extract might slow that enzyme down a bit. If that happened in real life, more formoterol could linger in your body, which could nudge up the usual side effects you might feel from this inhaler, things like a faster or pounding heartbeat, shakiness or tremor, jitteriness, or a headache.

The important part: this is only test-tube evidence and hasn't actually been seen in people, so I wouldn't lose sleep over it. Eating grapes is not a concern. If you're thinking about a concentrated grape seed supplement, just run it by your pharmacist or doctor first.

Fomoterol also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

It is unclear if grape juice or grape seed extract inhibits CYP2C9; research is conflicting.
In vitro evidence shows that grape seed extract or grape juice might inhibit CYP2C9 enzymes. However, a small pharmacokinetic study in healthy adults shows that drinking 8 ounces of grape juice once does not affect the clearance of flurbiprofen, a probe-drug for CYP2C9 metabolism. The effects of continued grape juice consumption are unclear.

Here's the honest picture for formoterol (Foradil). The concern on file is that grape juice or grape seed extract might block a liver enzyme called CYP2C9. If it did, drugs cleared by that enzyme could build up a little. In theory, more formoterol hanging around could mean stronger beta-agonist effects like a faster or pounding heartbeat, jitteriness, or shakiness.

That said, I wouldn't lose sleep over this one. The evidence is weak (lab and animal data), and a small human study showed grape juice didn't change how the body cleared a similar drug. Formoterol works as-is, so it's not a prodrug needing activation. Enjoy grapes as food freely. If you're considering a concentrated grape seed supplement, just run it by your pharmacist or doctor first.

What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
