Methadone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with grape seed extract and your methadone. Lab studies (done in test tubes, not in people) hint that concentrated grape seed extract might slow down an enzyme called CYP2D6, which is one of the pathways your body uses to clear methadone. If that enzyme works slower, methadone could in theory build up a bit. Since methadone is an active drug (not a prodrug that needs switching on), more of it in your system could mean stronger effects like extra drowsiness, slowed breathing, or constipation.
That said, this is only a theoretical concern and hasn't actually been seen in humans, and eating grapes or drinking grape juice is very unlikely to matter. Concentrated supplements are the question mark. Methadone is a sensitive medicine, so check with your pharmacist or doctor before starting a grape seed product.

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the honest picture: your body uses an enzyme called CYP3A4 to help clear methadone. Grape seed extract may tug on that enzyme, but the research pulls in two directions. Lab (test tube) studies hint it might slow CYP3A4, which could let methadone build up and raise side effects like heavy drowsiness, slowed breathing, or constipation. Animal studies suggest the opposite, that it might speed up the enzyme, which could weaken methadone and risk withdrawal-type symptoms.
The key thing to know is that none of this has actually been seen in people, so it stays theoretical for now. Still, methadone is a drug where small shifts matter, so please check with your pharmacist or doctor before adding a grape seed supplement. Eating grapes as food isn't a concern.

What Is Methadone Used For?

Methadone treats two main problems. Some products are used for severe, persistent pain that needs daily, around-the-clock opioid treatment when other options are not adequate. Some products are used for opioid addiction (heroin or other morphine-like drugs), as detoxification or as maintenance treatment along with social and medical services.
Not every product is approved for every use. Methadone tablets, oral solution and oral concentrate (Intensol) list both pain and opioid addiction. Diskets, Methadose Dispersible tablets and the Methadose and Methadone Hydrocloride oral concentrates are labeled only for opioid addiction. Methadone injection is for severe pain, and for short-term treatment of opioid dependence in people who cannot take medicine by mouth, such as hospitalized patients. It is not approved for outpatient opioid dependence treatment. Methadone is not meant to be used as an as-needed pain reliever.
