Tolterodine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Tolterodine (Detrol). Your body uses an enzyme called CYP2D6 to help break this medicine down. Lab testing hints that concentrated grape seed extract might slow that enzyme. If it does, tolterodine could clear a little slower and build up, which may nudge up its usual side effects: dry mouth, constipation, blurry vision, drowsiness, or trouble fully emptying your bladder.
The important part: this is only a theoretical finding from test-tube studies, and it has not been seen in actual people. Eating grapes or drinking grape juice almost certainly won't cause this. If you're thinking about a daily grape seed extract supplement alongside Detrol, just run it by your pharmacist or doctor first to be safe.

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the gist for Tolterodine (Detrol). Your body uses a liver enzyme called CYP3A4 to help clear this medicine, and grape seed extract may bump into that same enzyme. The tricky part is that the research pulls in two directions: some lab studies hint that grape seed extract slows CYP3A4 down (which could let tolterodine build up and bring more dry mouth, constipation, blurry vision, or dizziness), while animal studies suggest it might speed CYP3A4 up (which could weaken how well your bladder symptoms stay controlled).
Honestly, this is all early test-tube and animal data, and nothing like it has shown up in real people yet. Eating grapes shouldn't be a worry. If you're thinking about a concentrated grape seed supplement, just run it by your pharmacist or doctor first.
