Isoniazid, Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Gravel Root interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with combining Gravel Root and Rifamate (the isoniazid plus rifampin combo). Gravel Root contains plant compounds called pyrrolizidine alkaloids, which can be hard on the liver. Your body actually turns these into their toxic forms using a liver enzyme called CYP3A4. Rifampin is a strong activator of that same enzyme, so in theory it could speed up the making of those harmful byproducts.

Why this matters here: both isoniazid and rifampin already carry a real risk of liver injury on their own. Adding an herb that may stress the liver could stack on more risk. Watch for signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. To be fair, this interaction is theoretical and based only on lab work, not human studies. Still, given that your medications are already tough on the liver, I'd skip Gravel Root and check with your doctor or pharmacist first.
