Here's the simple version. Guar gum is a soluble fiber that swells into a thick gel in your stomach. That gel can grab onto pills and slow or lower how much of them your body actually soaks up. With macitentan (Opsumit), the concern is that taking it too close to a guar gum supplement could trap some of the dose, so less of it gets into your bloodstream.

For a medicine you rely on to keep the blood vessels in your lungs open, getting less than your full dose isn't ideal, since weaker effect could mean poorer control of your pulmonary hypertension. The evidence here is moderate and comes mostly from other drugs, not macitentan itself, so don't panic. A simple fix is spacing them apart (take macitentan first, then guar gum 30 to 60 minutes later). Please run your timing by your pharmacist or doctor first.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.

In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
