Macitentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.

Here's the simple version. Your body relies on a liver enzyme called CYP3A4 to break down Macitentan. In lab dish studies, devil's claw appeared to slow that same enzyme down a bit. If that were to happen in real life, your Macitentan could clear more slowly and build up, which might lead to more of its usual side effects, things like headache, stuffy nose, swelling in the legs or ankles, low blood pressure, or feeling unusually tired.

The honest part: this is only a theoretical concern from test-tube research, and it has never been shown to happen in people. So I wouldn't panic. Still, since PAH is serious, please check with your pharmacist or doctor before adding devil's claw rather than starting or stopping anything on your own.

About Macitentan

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a serious condition where the blood pressure in the arteries supplying the lungs is abnormally high. The goal is to slow down how quickly the disease gets worse and to lower the chances of being hospitalized because of it.

In clinical studies, it was shown to be effective in adults with mostly moderate-to-severe PAH symptoms. Patients in those studies had PAH caused by unknown or inherited reasons, connective tissue disorders, or congenital heart disease with repaired heart shunts.
