Higenamine and Lenacapavir

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Lenacapavir

Lenacapavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Lenacapavir (Sunlenca), your long-acting HIV medicine. Your body partly clears it using a liver enzyme called CYP3A4. In a test-tube study, higenamine slowed that enzyme down by about 21%. If that held true in people, lenacapavir might clear a bit more slowly and its levels could creep up.

Now the honest part: this is only lab-dish data, and it has not been shown to happen in real patients. So I wouldn't lose sleep over it. Lenacapavir isn't a prodrug, so higher levels would mean more drug effect, not less protection. My bigger worry with higenamine is its stimulant punch on your heart rate and blood pressure. Please run it by your pharmacist or HIV doctor before combining the two.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

“Lenacapavir (Sunlenca) treats HIV that has resisted other regimens, so keeping its blood level steady is not optional, and its list of 138 interacting supplements deserves genuine care. Nearly all of them, 121 ingredients, can shift how the body handles lenacapavir through the CYP3A4 enzyme, either pushing its level up along with side…”

About Higenamine

What is Higenamine?

  • Weight loss and fat burning
  • Pre-workout energy and performance
  • Appetite suppression
  • Athletic stimulant
  • Traditional medicine remedies

Higenamine is a plant-based stimulant compound added to many weight-loss and pre-workout supplements, but there is very little human evidence that it works and real concerns about heart and blood pressure effects. It is banned in sports by the World Anti-Doping Agency (WADA), and product labels often list inaccurate amounts, so most people should approach it with caution and talk to a pharmacist or doctor first.

About Lenacapavir Oral

What Is Lenacapavir Oral Used For?

Brand name: Sunlenca

Sunlenca tablets are used in two situations: to treat HIV-1 infection in adults whose virus has become resistant to multiple antiretroviral drugs and whose current regimen is failing — always in combination with other antiretrovirals — and as a short oral loading dose to start the BIXLENVO complete HIV-1 treatment regimen in adults who are already virologically suppressed (virus undetectable) and switching from a stable antiretroviral regimen.

Yeztugo tablets are used as part of HIV-1 pre-exposure prophylaxis (PrEP) — helping HIV-negative people reduce their risk of getting HIV-1. The oral tablet form is used during initiation of Yeztugo, and also as a temporary oral bridge if a scheduled injection must be delayed more than two weeks.
