Why this applies to Lenacapavir

Lenacapavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Breadfruit interacts with.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist for Sunlenca (lenacapavir), the long-acting HIV medicine. Your body partly clears lenacapavir using a liver enzyme called CYP3A4. Breadfruit leaf products may gently slow that same enzyme, at least in lab and animal studies. If it did so in a person, lenacapavir could build up a little, which in theory might mean more of its usual effects, like injection-site reactions or nausea.

I want to be honest though: this is theoretical and graded as minor. We don't have human data showing it actually matters, and eating breadfruit as food almost certainly won't move the needle. Concentrated leaf teas or extracts are the bigger question mark. Since lenacapavir is an HIV drug, please run any breadfruit supplement past your pharmacist or doctor first.

About Lenacapavir Oral

What Is Lenacapavir Oral Used For?

Brand name: Sunlenca

Lenacapavir tablets are sold under two brand names, and each has its own use.

  • Sunlenca is used with other HIV medicines to treat HIV-1 infection in heavily treatment-experienced adults with multidrug resistant HIV-1. It is for people whose current regimen is failing because of resistance, intolerance, or safety concerns.
  • Sunlenca tablets are also used for oral loading when starting Bixlenvo (bictegravir/lenacapavir). This is for adults who are virologically suppressed on a stable regimen with no known or suspected resistance to Bixlenvo's components.
  • Yeztugo is used for HIV-1 pre-exposure prophylaxis (PrEP). Its tablets are part of the starting doses and can be used as a short-term bridge if an injection is delayed.
