Why this applies to Rivaroxaban

Rivaroxaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the concern with mixing higenamine and Xarelto (rivaroxaban). Your body relies partly on an enzyme called CYP3A4 to break down and clear rivaroxaban. In lab dish testing, higenamine slowed that enzyme down a little (about 21%). If that happened in real life, rivaroxaban could build up slightly and act stronger than intended. Since rivaroxaban is a blood thinner, the main worry would be more bleeding, things like easy bruising, nosebleeds, bleeding gums, or blood in the urine or stool.

I want to be honest though: this is only a theory from test-tube research and hasn't been shown to actually happen in people. The bigger issue is that higenamine itself can raise heart rate and blood pressure. Please check with your pharmacist or doctor before combining them.

Here's the gist for Xarelto (rivaroxaban), which is a blood thinner that works by blocking a clotting factor in your blood. On its own, it already raises your chance of bleeding and bruising. Higenamine may add a little to that effect, because in animal studies it kept platelets (the cells that help clots form) from sticking together. Stacking the two could, in theory, tip you toward more bleeding-type problems: easier bruising, nosebleeds, bleeding gums, or bleeding that's slower to stop.

I want to be honest about the strength of this, though. It is theoretical and based on lab and animal work, not human studies. Even so, since higenamine shows up in pre-workout and weight-loss products with iffy labeling, it's worth a quick chat with your pharmacist or doctor before combining them.

Higenamine is a plant-based stimulant compound added to many weight-loss and pre-workout supplements, but there is very little human evidence that it works and real concerns about heart and blood pressure effects. It is banned in sports by the World Anti-Doping Agency (WADA), and product labels often list inaccurate amounts, so most people should approach it with caution and talk to a pharmacist or doctor first.

About Rivaroxaban

What Is Rivaroxaban Used For?

Brand name: Xarelto

Rivaroxaban tablets are used to reduce the risk of stroke in adults with nonvalvular atrial fibrillation (an irregular heart rhythm), treat deep vein thrombosis (DVT, a blood clot in a vein) and pulmonary embolism (PE, a blood clot in the lungs), and lower the chances of these clots coming back. They also help prevent clots after hip or knee replacement surgery, protect hospitalized patients at risk for clots, and — in combination with aspirin — reduce the risk of major cardiovascular events in adults with coronary artery disease (CAD) or peripheral artery disease (PAD).

Rivaroxaban tablets are also approved for treating VTE (venous thromboembolism, blood clots in veins) and preventing recurrence in pediatric patients from birth to under 18 years, and for preventing clots in children aged 2 and older with congenital heart disease after the Fontan procedure. Rivaroxaban for oral suspension (available as Rivaroxaban Granule and Xarelto oral suspension) is specifically indicated for these two pediatric uses.
