Belladonna, Ergotamine Tartrate, Phenobarbital falls under CNS DEPRESSANTS — the group Hops interacts with.

The medication you're asking about (Bellergal-S) contains phenobarbital, which is a barbiturate that already makes many people feel sleepy or slowed down. Hops are used mainly to help with sleep and mild anxiety, and a few animal studies suggest they may have a calming, sedative effect too. So the concern here is simple: putting two things that relax the brain together could add up.
In real life, that might mean extra drowsiness, grogginess, dizziness, or feeling foggy and slow to react, which matters for driving or anything that needs focus. I'll be honest that this is a theoretical caution based on how these compounds work, not strong human proof. Still, since phenobarbital is potent, it's worth running it past your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Belladonna, Ergotamine Tartrate, Phenobarbital also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Bellergal-S combines belladonna, ergotamine, and phenobarbital, and one of those (ergotamine) is partly cleared by a liver enzyme called CYP3A4. The thinking here is that hops might speed up that enzyme a little, which in theory could lower ergotamine levels and make it slightly less effective. But honestly, this is on very shaky ground. The evidence comes from lab and animal studies, and a human study using a similar enzyme test drug found only about a 7.6% change, which is too small to matter.
So for everyday use, I wouldn't expect hops to noticeably change how your Bellergal-S works. Still, since this medicine is fairly potent and ergot drugs need care, it's worth a quick mention to your pharmacist or doctor before adding a hops supplement.
