Here's the deal with Nextstellis, which is a birth control pill that pairs drospirenone with estetrol (a plant-like estrogen). Hops contain a flavonoid called 8-prenylnaringenin that can act like a weak estrogen and may bump up against the same estrogen receptors your medication targets. In theory, large concentrated amounts of hops could compete a bit with the estetrol part and slightly muddy how it works.

I want to keep this in perspective. This concern comes only from lab studies, not from real people, and most hops products contain only tiny amounts of that compound. A regular sleep or relaxation supplement is unlikely to affect your pill or its reliability. Still, since this is birth control, it's worth a quick check with your pharmacist before adding concentrated hops.

Hops extract does not seem to affect the metabolism of CYP1A2 substrates.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

If you take Nextstellis (the birth control pill with drospirenone and estetrol) and you're curious about adding hops for sleep, here's the honest picture. The concern on file is that compounds in hops might slow down a liver enzyme called CYP1A2. In a test tube, hops flavonoids did look like they could block that enzyme. But when researchers actually gave standardized hops to real people, it didn't meaningfully change how that enzyme worked.

So in practice, this interaction is rated minor and unlikely, and the evidence behind it is weak (lab data, not human results). I wouldn't expect hops to throw off your pill's effectiveness or cause new problems. Just know hops can make you drowsy on their own. If you're using hops regularly, it's always smart to run it by your pharmacist or doctor first.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the situation with Nextstellis (drospirenone and estetrol). The estrogen part is processed by a liver enzyme called CYP3A4, and there's a theory that hops could nudge that enzyme to work a little faster. If that happened, your birth control could be cleared slightly quicker, which in theory could mean a touch less hormone in your system.

But honestly, the evidence here is very thin. It comes mostly from lab and animal studies, and a human study using a hops extract changed a test drug's level by under 8 percent, which is too small to matter. I would not expect breakthrough bleeding or reduced pill protection from normal hops use. Still, since this is your contraception, it's worth a quick check with your pharmacist or doctor.
