Enasidenib falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hops interacts with.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

Here's the short version on combining hops with Idhifa (enasidenib): the worry is mostly theoretical. In a test tube, certain hops flavonoids appeared to slow down a liver enzyme called CYP1A2, which can play a role in clearing enasidenib. On paper, blocking that enzyme could let the drug build up a little and, in theory, nudge up side effects like nausea, low appetite, or changes in your blood counts.
But here's the reassuring part: in an actual human study, a standardized hops extract did not change how the body handled a CYP1A2 test substance. So real-world interaction looks unlikely. Still, enasidenib is a serious leukemia medicine, so please run any hops supplement past your oncology team or pharmacist before starting it.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your leukemia medicine Enasidenib (Idhifa) is partly cleared by a liver enzyme called CYP3A4. There's a theory that hops can nudge that enzyme to work a little faster. If that happened, your body might break down Enasidenib slightly quicker, which could in theory weaken how well it fights the cancer cells. The direction here is toward less drug, not more, since Enasidenib is the active drug itself and not a prodrug.
The honest part: this is based only on lab and animal studies, and a human study found just a tiny 7.6% drop with a similar drug, which isn't enough to matter. So I wouldn't expect real trouble. Still, with a cancer treatment this important, please run hops past your oncologist or pharmacist before adding it.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
