Why this applies to Fostamatinib

Fostamatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the thing to know about fostamatinib (Tavalisse): your body turns it into its active form, called R406, and the CYP3A4 enzyme then helps clear that active piece out. Lab and animal studies hint that hops compounds (lupulones) might nudge CYP3A4 to work a little faster. If that happened, it could theoretically lower fostamatinib levels and make it slightly less effective at keeping your platelet count up.

The good news is that this is mostly a theoretical concern. The one human study on hops showed only a tiny change (under 8%) in a similar drug, which isn't enough to matter in real life. Food amounts, like beer, are even less of an issue. Still, since your platelet count needs to stay steady, it's worth a quick chat with your pharmacist or doctor before adding a hops supplement.

What Is Fostamatinib Used For?

Brand name: Tavalisse

Tavalisse (fostamatinib) is used to treat thrombocytopenia — dangerously low platelet counts — in adults with chronic immune thrombocytopenia (ITP). It is specifically for people whose platelet counts have not improved enough with at least one earlier ITP treatment.

The goal is to raise platelet counts high enough to reduce the risk of serious bleeding. If platelet counts do not respond sufficiently after 12 weeks, the medication is typically stopped.
