Tramadol, Acetaminophen falls under CNS DEPRESSANTS — the group Hops interacts with.
Some animal research shows that hops has sedative effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: hops may have a mild calming, sleep-promoting effect, and Tramacet (which pairs tramadol with acetaminophen) already brings its own drowsiness to the table. The tramadol part especially can leave you feeling sleepy, dizzy, or a little foggy. Stacking hops on top could add to that sedation, so you might feel groggier than usual, slower to react, or unsteady on your feet.
I want to be honest about the evidence though. This concern is mostly theoretical, based on how hops behaves in animal studies, not strong human proof. For many people it won't be a big deal, but it's worth mentioning to your pharmacist or doctor, especially if you'll be driving or already feel drowsy on Tramacet.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 3Tramadol, Acetaminophen also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Good news here: the concern about hops slowing down the CYP1A2 enzyme really hasn't held up. In a lab dish, certain hops flavonoids looked like they might block CYP1A2, but when researchers gave people a standardized hops extract, it did not change how the body handled caffeine, a drug that relies on that same enzyme. So for your Tramacet (tramadol plus acetaminophen), this particular pathway interaction looks very unlikely to cause a real problem.
One thing I'd still mention: hops can make you sleepy on their own, and tramadol can too. Stacking them might leave you more drowsy or foggy than usual. If you plan to use both regularly, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist for your Tramacet (tramadol with acetaminophen). The tramadol part is interesting because it's partly a prodrug, meaning your liver has to activate it to give the strongest pain relief. The lab and animal data suggest hops might slightly speed up one liver enzyme (CYP3A4). In theory, faster clearance could mean a touch less pain relief, but the human study on this showed only a tiny change (under 8%), which isn't enough to notice.
So practically speaking, this is a very minor, mostly theoretical concern. The one thing I'd actually watch is that both hops and tramadol can make you drowsy, so together you may feel more sleepy or foggy. Please run it by your pharmacist or doctor before combining them.
