Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the short version: Combogesic combines acetaminophen and ibuprofen, two common pain and fever reducers. In a test-tube study, horny goat weed slowed down a liver enzyme called CYP1A2. If that happened in your body, the idea is that drugs cleared by that enzyme could build up a little and hit harder. Neither acetaminophen nor ibuprofen is a prodrug, so the theory points toward slightly stronger effects, not weaker ones.

That said, this is lab-only evidence and has not been seen in real people, so I wouldn't lose sleep over it. Still, if you take this combo regularly, watch for things like stomach upset, heartburn, or feeling unusually drowsy, and check with your pharmacist or doctor before pairing them.

In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the thing to keep in mind with Combogesic (the acetaminophen and ibuprofen combo): the ibuprofen part is an NSAID, and NSAIDs already nudge up your bleeding risk a little on their own by affecting how your platelets clump together. In lab and animal studies, horny goat weed seems to do something similar, slowing platelet activity. So in theory, pairing the two could add up and make bleeding or bruising a bit more likely.

I want to be honest though: this has only been seen in test tubes and animals, never confirmed in people, so it's more a caution than a proven problem. Still, watch for easy bruising, nosebleeds, or stomach upset, and check with your pharmacist or doctor before combining them, especially if you take blood thinners or have heart trouble.
