Horny Goat Weed and Amlodipine Benzoate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Horny Goat Weed interacts with whole categories of medication. Amlodipine Benzoate falls into 2 of those categories, so it can interact with Horny Goat Weed in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Amlodipine Benzoate

Amlodipine Benzoate falls under ANTIHYPERTENSIVE DRUGS — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the risk of hypotension.
Laboratory research suggests that horny goat weed might have hypotensive effects. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the situation with amlodipine (Katerzia) and horny goat weed. Amlodipine is a calcium channel blocker that lowers your blood pressure by relaxing blood vessels. The concern is that horny goat weed might also nudge blood pressure downward, so stacking the two could in theory add up and push your pressure too low.

If that happened, you might feel dizzy, lightheaded, unusually tired, or notice your heart racing, especially when you stand up quickly. I want to be honest though: this is based only on lab and animal research, and it has not actually been reported in people. So it's a possible concern, not a proven one. Before adding this herb, just run it by your pharmacist or doctor so they can keep an eye on your numbers.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Amlodipine Benzoate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body clears amlodipine using a liver enzyme called CYP3A4. In test-tube studies, horny goat weed seems to slow that same enzyme down. If that held true in people, your body might break amlodipine down more slowly, letting more of it build up. That could nudge its blood-pressure-lowering effect stronger and make side effects more likely, things like swollen ankles, dizziness, flushing, headache, or feeling unusually tired.

The honest part: this is only seen in lab studies, not in real patients, so it's more theory than proven concern. Still, since amlodipine affects your blood pressure, it's worth a quick chat with your pharmacist or doctor before combining them. Don't change your dose on your own.

What is Horny Goat Weed?

Horny Goat Weed
  • Erectile dysfunction
  • Low libido
  • Fatigue and low energy
  • Bone health
  • Joint and menopausal symptoms
  • General 'vitality' or performance

Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. While short-term use seems generally tolerated by some people, quality and safety are not well studied, so talk to your pharmacist or doctor before trying it—especially if you have heart problems or take other medicines.

About Amlodipine

What Is Amlodipine Used For?

Brand name: Katerzia

Amlodipine lowers high blood pressure (hypertension). Lowering blood pressure reduces the risk of strokes and heart attacks. It can be used alone or with other blood pressure or angina medicines.

It is also used for chronic stable angina, vasospastic angina (Prinzmetal's or variant angina), and angiographically documented coronary artery disease in patients without heart failure or an ejection fraction below 40%. The Sdamlo oral solution label lists the high blood pressure use for adults and children 6 years and older, and the coronary artery disease uses for adults.
