In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the situation with Eliquis (apixaban) and horny goat weed. Apixaban is already a blood thinner that blocks a clotting protein called factor Xa. In lab dishes and animal studies, horny goat weed has shown a separate mild ability to keep platelets from clumping together. Stack those two together and the concern is simply that your blood could become a little too slow to clot.

The honest part: this has only been seen in test tubes and animals, never reported in real people, so the worry is mostly theoretical. Still, with a blood thinner, extra bleeding is worth respecting. Watch for easy bruising, nosebleeds, bleeding gums, or unusually dark stools. Before adding this herb, run it by your pharmacist or doctor first.

Apixaban also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist with Eliquis (apixaban): your body partly clears it using a liver enzyme called CYP3A4. In test-tube studies, horny goat weed appears to slow that same enzyme down. If that were to happen in real people, less apixaban would be cleared, so more could build up in your blood. With a blood thinner, that means a higher chance of bruising, nosebleeds, or more serious bleeding.

I'll be honest with you though: this concern comes only from lab research, not from studies in actual patients, so it's theoretical at this point. Apixaban is not a prodrug, so the direction here is more drug effect, not less. Because bleeding risk is nothing to gamble with, please check with your pharmacist or doctor before pairing these.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
