The pill you're asking about combines aspirin with hydrocodone. The aspirin part is the one that matters here, because aspirin already thins the blood a bit by keeping platelets from clumping together. In lab dishes and animal studies, horny goat weed seems to do something similar, slowing how platelets stick. Stacked on top of aspirin, the worry is a little extra bleeding risk, like easier bruising, nosebleeds, or bleeding that takes longer to stop.

I'll be honest though: this has only been seen in test tubes and animals, never confirmed in people, so it's more theoretical than proven. Still, if you take this aspirin combo regularly, run the horny goat weed by your pharmacist or doctor before adding it, especially before any surgery or dental work.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

This combo product pairs aspirin with hydrocodone, an opioid pain reliever. Your liver uses an enzyme called CYP3A4 to help break hydrocodone down. In test-tube studies, horny goat weed seemed to slow that same enzyme. If that happened in a real person, hydrocodone could clear more slowly and build up, which might mean stronger effects like extra drowsiness, dizziness, constipation, or slowed breathing.

Here's the reassuring part: this signal comes only from lab research, not from studies in people, so we honestly don't know if it matters in real life. Still, since opioids are nothing to gamble with, I'd run this past your pharmacist or doctor before pairing the two, rather than guessing.
