Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist for combining horny goat weed with your estrogen. Part of how your body clears estrogen runs through a liver enzyme called CYP1A2. In lab dish studies (not in actual people), an extract of horny goat weed slowed that enzyme down. If that were to happen in your body, estrogen could clear more slowly and build up a bit, which in theory might mean stronger effects or more side effects like breast tenderness, bloating, nausea, headaches, or mood changes.

The honest takeaway: this is only a theoretical concern based on test-tube work, so don't be alarmed. Still, because estrogen carries its own risks, it's worth a quick chat with your pharmacist or doctor before adding this herb. Please don't change your estrogen dose on your own.

Here's the simple version: horny goat weed seems to have some estrogen-like activity of its own, and a little human research suggests it can nudge estrogen blood levels up. So pairing it with your prescription estrogen could, in theory, stack on top of what your medication is already doing. Think of it as adding a bit more estrogen effect than you signed up for.

If that happens, you might notice more of estrogen's usual effects, like breast tenderness, bloating, headaches, or spotting. I want to be honest though: this is based on lab work and limited studies, not strong proof, so it may not happen at all. Since estrogen carries real risks, please run this by your pharmacist or doctor before combining the two.

If that same thing happened in your body, estrogen could hang around a bit longer and its effects could build up. In practice that might mean more of the usual estrogen-related complaints, things like breast tenderness, bloating, nausea, headaches, or spotting.

I want to be honest about how thin this evidence is. This is a theoretical concern from test-tube work only, and it has never been shown to happen in humans. So I wouldn't lose sleep over it, but I also wouldn't add the supplement on your own. Run it by your pharmacist or doctor first so they can look at your full picture.
