Galantamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist: your body partly relies on a liver enzyme called CYP3A4 to clear galantamine. In test-tube studies, horny goat weed appears to slow that same enzyme down. If that happened in people, galantamine could hang around longer and build up, which may mean stronger effects and more side effects, like nausea, vomiting, diarrhea, loss of appetite, dizziness, or a slower heart rate.

I want to be honest about the evidence, though. This is only lab and theory work, and it has not been shown in actual people, so the real-world risk is uncertain and probably small. Galantamine is not a prodrug, so the direction here points to more drug effect, not less. Still, please check with your pharmacist or doctor before pairing these.

Galantamine — available as extended-release capsules, tablets, and an oral solution — is used to treat mild to moderate dementia caused by Alzheimer's disease. All supported formulations share this indication.

It does not cure or slow the underlying disease, but it can help support cognitive function in people with early to moderate-stage Alzheimer's. Its benefits may lessen as the disease progresses and more brain cells are lost.
