Hydrocodone, Ibuprofen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Horny Goat Weed interacts with.
In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of Vicoprofen we're focused on here is the ibuprofen part. Ibuprofen is an NSAID, and like all NSAIDs it already makes platelets a little less sticky, which can nudge up your odds of bruising or bleeding. Horny goat weed has shown, in lab and animal studies only, a similar ability to slow platelets down. Stack those two effects and, in theory, you could see more bleeding-type problems like easy bruising, nosebleeds, or stomach irritation.
That said, this has never actually been reported in people, so the evidence is weak. Still, if you take blood thinners, have ulcers, or notice unusual bruising, check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Vicoprofen pairs hydrocodone (an opioid) with ibuprofen. The hydrocodone part is partly handled by a liver enzyme called CYP3A4. In lab dishes, horny goat weed slowed that enzyme down, which in theory could let hydrocodone build up a bit more than usual. If that happened, you might notice stronger opioid effects like extra drowsiness, dizziness, constipation, or slower, shallower breathing.
Here's my honest take: this is theoretical only, seen in test tubes and not in real people, so I wouldn't lose sleep over it. Still, hydrocodone is nothing to gamble with. Before combining the two, please check with your pharmacist or doctor, especially if you already feel groggy on this medicine.
