Hydroflumethiazide, Reserpine falls under ANTIHYPERTENSIVE DRUGS — the group Horny Goat Weed interacts with.
Laboratory research suggests that horny goat weed might have hypotensive effects. This effect has not been reported in humans.

The medicine you asked about combines two blood pressure lowerers: hydroflumethiazide (a water pill) and reserpine (which calms certain nerve signals). Both already work to bring your blood pressure down. Horny goat weed may, on its own, have a mild blood pressure lowering effect too, so stacking it on top could in theory push your numbers a little lower than intended.
If that happened, you might feel lightheaded, dizzy, tired, or faint when you stand up quickly. I'll be honest though: this concern comes only from lab and animal studies, and it has not been seen in real people. So it's worth a heads up, not a panic. Before trying horny goat weed, just run it by your pharmacist or doctor first.
