Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for mannitol (Osmitrol or Bronchitol). This medicine pulls extra fluid out of your tissues and into your urine, which can lower your blood volume and, along with that, your blood pressure. The concern on file is that horny goat weed may have its own mild blood-pressure-lowering effect. Stacked together, the worry is that your pressure could dip a little too low, leaving you feeling lightheaded, dizzy, or faint.

I want to be honest with you though: this is a theoretical flag based only on lab and animal research, not anything reported in real people. So it's not a reason to panic. Still, since mannitol is given in serious medical situations, it's worth mentioning any supplements you take to your doctor or pharmacist.

From our pharmacist’s overview on the Mannitol page

“American hellebore tops my worry list for anyone on mannitol (Osmitrol, Bronchitol), an osmotic diuretic used to draw fluid off the brain or, when inhaled, to clear mucus in cystic fibrosis. The plant's alkaloids drop blood pressure and slow the heart, a dangerous match for a drug that already shifts fluid and pressure quickly.”

  • Erectile dysfunction
  • Low libido
  • Fatigue and low energy
  • Bone health
  • Joint and menopausal symptoms
  • General 'vitality' or performance

Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. While short-term use seems generally tolerated by some people, quality and safety are not well studied, so talk to your pharmacist or doctor before trying it—especially if you have heart problems or take other medicines.
