Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist for Perampanel (Fycompa). Your body relies partly on a liver enzyme called CYP1A2 to clear this seizure medicine out. In test-tube studies, horny goat weed slowed that same enzyme down. If that happened in a real person, perampanel could build up a bit more than expected, which might mean stronger sedation, dizziness, trouble with balance, or those mood and irritability changes this drug is already known for.

I want to be honest about the strength here: this is only lab data, and it has not been seen in actual patients, so it's more of a theoretical caution than a proven problem. Still, since perampanel already makes a lot of folks drowsy, please run it by your pharmacist or doctor before adding this herb.

In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to clear perampanel (Fycompa). In test-tube studies, horny goat weed seems to slow that same enzyme down. If that happened in people, perampanel could build up a bit and hit harder than expected. With this particular seizure medicine, that usually shows up as more drowsiness, dizziness, trouble with balance or coordination, and in some cases irritability or mood changes.

I want to be honest about the strength of this, though. It comes only from lab work, not from real patients, so it's theoretical at this point. Still, because perampanel already causes sedation, I'd play it safe. Please check with your pharmacist or doctor before combining the two rather than adjusting anything yourself.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
