CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Rifabutin
Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses an enzyme called CYP3A4 to help break down rifabutin. In lab dishes, horny goat weed seems to slow that enzyme down. If that happened in real people, rifabutin could clear out of you a little slower and build up, which might mean more of its usual side effects, things like nausea, stomach upset, an orange-red tint to urine and tears, joint aches, or changes in your blood counts.
The important part is that this is only seen in test tube and animal studies, not in actual patients, so it stays theoretical for now. Since rifabutin is doing serious work fighting infection, I wouldn't combine it with this herb without checking first. Please run it by your pharmacist or doctor before adding horny goat weed.

Rifabutin capsules are used to prevent disseminated MAC disease in people with advanced HIV infection. MAC is an infection caused by Mycobacterium avium complex bacteria that spreads through the body.
It is not a preventive for tuberculosis. It must not be used for MAC prevention in anyone with active tuberculosis.
