Riociguat falls under ANTIHYPERTENSIVE DRUGS — the group Horny Goat Weed interacts with.
Laboratory research suggests that horny goat weed might have hypotensive effects. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about Riociguat (Adempas): its whole job is to relax and widen blood vessels, which naturally lowers blood pressure. Low blood pressure is actually one of its most common side effects. Horny goat weed enters the picture because lab and animal research hints that it might also nudge blood pressure downward. Stack the two together and, in theory, you could get a bigger drop than expected.
If that happened, you might feel dizzy, lightheaded, weak, or even faint, especially when standing up quickly. I'll be honest with you though: this blood-pressure-lowering effect of horny goat weed has only shown up in the lab, not in real people, so it's more of a caution than a proven problem. Still, please check with your doctor or pharmacist before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Riociguat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Adempas (riociguat). Your body partly clears this drug using a liver enzyme called CYP3A4. In test tube studies, horny goat weed seemed to slow that enzyme down. If that happened in real life, riociguat could clear more slowly and build up, which may push its blood-pressure-lowering effect too far. That can show up as dizziness, lightheadedness, headache, an upset stomach, or fainting, especially when you stand up.
The reassuring part: this is theoretical, seen only in lab work, never confirmed in people. Riociguat is a sensitive medicine, though, so I wouldn't combine them on your own. Please run it by your pharmacist or doctor first.

Riociguat — sold as Adempas and under the generic name Riociguat — is used to treat adults with two conditions that cause dangerously high blood pressure in the arteries of the lungs.
- Chronic Thromboembolic Pulmonary Hypertension (CTEPH): Used after surgical treatment, or when surgery is not an option, to improve exercise capacity and functional class.
- Pulmonary Arterial Hypertension (PAH): Used to improve exercise capacity, functional class, and to delay the condition from getting worse. It can be used alone or combined with certain other PAH medicines.
